Total Shoulder Replacement

Shoulder replacement is less commonly performed than replacement of the hip or knee but remains a very useful treatment for a variety of shoulder conditions. Osteoarthritis is much less common in the shoulder than in the knee or hip so replacement is more frequently performed for arthritic diseases such as rheumatoid arthritis and for painful conditions secondary to trauma such as fractures or rotator cuff dysfunction. Replacing the shoulder is a different matter from the main weight bearing joints of the lower limbs which can be thought of as .hard tissue joints.

The shoulder is more of a .soft tissue joint. in that the orientation and sufficiency of the soft tissues are what allows the joint to function properly. Typically the socket is replaced by a plastic socket component and the humeral head by a metal component which has a stem down the humeral shaft. The correct positioning of the humeral head in relation to the soft tissues such as the rotator cuff tendons is vital as if this is not correct then the joint surfaces may be fine but the shoulder will not function correctly. Patients are routinely referred to physiotherapy, initially for range of motion and later for strengthening and functional work.

Discs and Height

Physical activities change the nature of the intervertebral discs throughout the day and night. The volume and the height of discs can change by up to twenty percent, with a person.s height varying by anything from fifteen to twenty five millimetres. Most of the change in height happens during the first hour after getting up as the body weight loading is applied to the spine. During the night the opposite happens with the disc being unloaded so it sucks in water from the tissue around it until it reaches a balance of pressures on both sides.

This daily cycle of pressure variation changes the water content of the disc regularly and may be part of the explanation as to why back pain sufferers have more pain in the mornings due to the increased tension present in the disc complex. Resting or performing heavy work can cause changes in the hydration of the disc, carrying a twenty kilogram backpack for three hours has been shown to reduce the volume of the discs by 4.5 percent. The amount of water content in the disc does not achieve a specific resting level; it depends on how much loading is going on and for how long.
