Osgood-Schlatter Disease

Osgood-Schlatter disease is a condition affecting the tibial tuberosity (bony lump just below the kneecap) in children around eight to thirteen years old. It typically affects around 20 percent of athletic children of these ages as compared to less than 5 percent of nonathletic children. It tends to coincide with spurts of growth and is three times more likely in boys rather than girls. In young children the tibial tuberosity or tibial tubercle is formed of cartilage and only ossifies into bone later and it is postulated that repeated traction applied to this area can cause minor fractures or inflammation. This leads to pain on vigorous activities where the quadriceps are involved.

Pain, tenderness and swelling typically develop over the tubercle, exacerbated by activity, but the condition is self-limiting and as the growth plate areas fuse into bone the condition gradually settles. As the athletic young person perform sport, the stresses cause minor fractures in the tendon insertion area, which then heals with bony repair. When this is repeated many times the tibial tubercle may become very prominent and give long term problems with functional activities such as kneeling. Limitation of the aggravating activity or short-term immobilisation is required until the inflammation settles.

Simple Neck Exercises

Neck problems are very common with neck, head, upper back, shoulder and arm pain all possibly referred from the cervical spine discs, joints, ligaments and muscles. The cervical spine is a long column with a heavy weight (the head) balanced at one end, a process that demands good movements and strong muscular control to achieve effectively without pain. Neck pain is commonly secondary to poor posture over a long period of time such as working at an office desk or kitchen worktop, where the neck has to maintain one posture for much longer than is helpful.

It can be helpful to perform simple neck exercises every day to maintain the ranges of motion of all the cervical joints and other structures within normal limits. Different parts of the cervical spine are specialised for different movements and it is important to make sure that the specific movements for each joint are practised daily. The upper neck vertebrae are specialised for rotation as moving the head round to look at something is a very important ability, and there is a small amount of a nodding movement here also. Further down the neck the focus is more on forward and back movements functionally adapted for looking up and down.
