Supraspinatus Tendonitis of the Shoulder

The shoulder joint is controlled by a group of muscles known as the rotator cuff which originate from the scapula or shoulder blade and travel outwards until they become tendons and insert into the top of the arm bone. The cuff stabilises and holds the large ball of the arm bone in place on the small socket as the shoulder goes through its very large range of motion. Without the rotator cuff the large and powerful deltoid muscle would just pull the shoulder area upwards rather than lift the arm above the head. The supraspinatus muscle is one of the cuff group and its tendon runs just underneath the bony arch above the arm bone to attach to the outside.

If the shoulder is used repetitively for an unaccustomed activity the tendon can impinge on the bony arch above and become inflamed and painful. This is supraspinatus tendonitis, a condition which can be acutely painful or more chronically restricting for the use of the shoulder. Physiotherapy treatment consists of ultrasound or frictions to the tender area and correction of any shoulder muscle imbalances. Further treatment includes corticosteroid injections to reduce the inflammation and if severe enough, investigation to look for rotator cuff tendon tears.

Physiotherapy and Falls

Falls are a problem at any age but particularly so in elderly due to the frequency and the severe potential problems which can result. The incidence of fractured neck of femur (fractured hips) in the elderly is considerable and carries significant risk of long term disability or death in people who are already frail and at the edge of independence. Falling is a complex problem which demands a multidisciplinary approach from health professionals to have any chance of success. As we get older our muscular, balance, joint, vision and hearing systems all change for the worse and reduce the speed and amount of information and postural change we are able to understand and accommodate at any time.

Physiotherapists assess and manage falls problems by assessing the medical history and present status of the patient, getting a social history from them and assessing their mental and physical status. The physio checks the patient's ability to see clearly and their awareness of the space around them which is impaired in many medical conditions. The patient is asked to get up and their balance reactions assessed, after which the physio will get them to walk to check for gait abnormalities or balance problems in dynamic activities such as gait.
