Hip Dislocation

The hip is one of the largest and strongest joints in the body, being a deep ball and socket joint with powerful ligaments holding it all together. This means that dislocation is rare and only occurs in the event of significantly violent events such as motor vehicle crashes. In an accident the thigh may be forced into a position where the hip is levered out of its socket, tearing its surrounding ligaments and remaining out of socket. This causes extreme pain and patients have to be anaesthetised to relocate the joint. Treatment is complicated by the potential threat to the blood supply to the head of the femur.

Dislocation can also occur centrally, where the head of the femur is driven through the socket, fracturing it and ending up inside the pelvis. This injury usually needs internal fixation and significant time non weight bearing until the fractures heal. Normal dislocations are also treated with a period of non weight bearing to ensure the blood supply to the head of the femur, which was disrupted by the dislocation, restores itself. Once weight bearing is allowed the physiotherapist can instruct the patient in normal gait and exercises to restore the strength of the hip muscles.

Thumb Osteoarthritis

Osteoarthritis is the commonest degenerative joint disease in the world and affects hundreds of millions of mostly older people. The incidence of osteoarthritis is partly related to genetic factors but mostly the older a person is the more likely they are to have some arthritic joints. Some joints are more susceptible to osteoarthritic changes and more troublesome when they are affected by this condition. At the base of the thumb is the junction between the first metacarpal and one of the carpal or wrist bones, the trapezium, and this joint is known as the first carpo-metacarpal joint or CMC joint.

The CMC joint is a complex, saddle shaped joint and a very important one for the function of the hand. The thumb is a vital part of normal hand use and disturbance of the thumb can badly affect the use of the whole hand. The thumb has to rotate round so if can work opposite the fingers and if the joint become osteoarthritic this is restricted. A bony swelling may be obvious on the outer border of the hand below the thumb and the movement is obviously limited. Treatment includes joint mobilisations by a physiotherapist, splinting, injections and surgery.
