Physiotherapy Mobilisation Techniques

Physiotherapists use a large number of treatment techniques to manage the variety of conditions their patients present with. Joint problems make up a large proportion of a physio.s caseload and to treat these a number of treatment approaches have been developed which go under the general umbrella of mobilisation techniques. Joint mobilisations are a technique of stressing the joint manually by moving it through a proportion of its range and with a specific amount of force. Manipulation, meaning work done with the hands, includes the more vigorous manoeuvres which are commonly referred to as manipulation.

Initially a physiotherapist will fully assess the joint for the type of problem which is present such as primarily pain, pain and stiffness or primarily stiffness with little pain. Mobilisations for pain will be gentle and rhythmic to avoid aggravating an already irritable joint. If the pain is less severe and there is some stiffness the joint can be pushed a little harder. If stiffness is the main problem in the absence of pain then the joint may tolerate being pushed repeatedly to the end of its available movement in an attempt to increase its range of motion. Training is required to assess and treat a joint safely and effectively.

Time and Muscle Degeneration

Ageing is associated with decreases in both muscle bulk and strength, varying with different muscles in the body and with which side is the dominant one. The loss of the capacity of muscle is greater in men than in women and also in people who are less active physically. Hormone changes, reduced rates of protein production and poor dietary intake can all contribute. Physiotherapists can reverse this process by instituting a resisted exercise programme at the higher intensity level. Older people can respond to muscle training although older muscles do react less strongly to stimulation and take longer to recover and strengthen after exercise sessions.

Older muscles also suffer from a variety of other changes: a reduction in the ability to use energy to produce force; an increase in the fibrous tissue content which can increase stiffness. Along with the reduction in the ability to know where the joints are at any one moment the elderly suffer from slower reactions and poorer muscle control, with weaker and more easily tired muscles. This can lead in very elderly people to a loss of independence and increasing frailty. Perhaps because of its good blood supply, muscle is mostly able to regenerate itself and remain strong.
