Hypermobility

We are all born with a genetically determined type of fibrous tissue, the collagen material that makes us all or most of our ligaments, tendons, skin and discs. Our collagen varies greatly in the amount of extensibility it allows due partly due to the amount of elastic material included and the length of our tissues. Normal mobility varies from stiff jointed to very mobile and individuals who show very high degrees of flexibility of their joints are termed hypermobile. Hypermobility may be asymptomatic or can cause significant pain problems in other cases. Benign hypermobility syndrome BHS is diagnosed clinically by testing the patient's joint mobility.

The diagnostic tests are: the little finger can be pulled back to 90 degrees to the hand; with the wrist bent the thumb can be pulled back to touch the forearm; the elbows extend back beyond neutral; the knees extend back beyond neutral and the person can bend over with the knees straight and place their palms on the floor. Each joint gets a score of one if positive and this gives a score out of nine. The score indicates the degree of hypermobility the patient is exhibiting and helps decide whether the joint condition is relevant.

The Curves of the Spine

A quick look at anyone's spine from the side clearly indicates that the spine is not a dead straight structure but has a series of curve. There are four spinal curves, the cervical, thoracic, lumbar and sacral. The neck curve begins to develop when the young child starts to try and lift the head up to look around, while the lumbar curve may be secondary to an upright gait. During walking the curves vary in regular amounts and this may be so that the spine can absorb and release energy, allowing the curves to be increased and decreased rather than the spine just suffers compression. This makes the curve system a form of shock absorber.

During the gait cycles the large muscles of the abdomen and back are working hard to stop large changes in the spinal curves occurring as this would adversely affect stability, and it is these tendons that the energy is stored which would otherwise just be applied to the spine. Walking involves jarring up and down movements which could be harmful if they were transmitted up through the spine and arrived at the head. These forces are lessened by the absorption of forces which the curves provide.
