Bunions

Bunions is the term given to problems which occur with the joint of the big toe as it meets the foot, the first metatarsophalangeal joint or MTP joint. Problems with this joint may be familial with some inherited tendency but can also be do to poor foot use such as ill fitting footwear. Signs may start with an increased prominence at the inner border of the foot as the big toe drifts slightly towards the outside of the foot, eventually leading to rubbing of the prominence on the shoe in walking. Progression may occur as the deformity becomes worse and the joint itself becomes painful, with the big toe compressing the second toe and perhaps overriding or under riding it.

A highly arthritic joint can be very disabling, severely restrict the ability to walk very far and cause huge problems in choosing suitable footwear. Initial physiotherapy treatment can be provision of insoles, mobilisation techniques, gait correction, provision of walking aids and moderation of aggravating activities. More severe problems can be treated by joint injection, shaving of the joint surgically, removal of the joint to leave a fibrous union, fusion of the joint (arthrodesis) or replacement of the joint with a prosthesis.

Bone and Ageing

Bone loss occurs much more rapidly than the equivalent process in muscle tissues, with reductions in hormone production and in activity playing a part. After the menopause, the reduction in hormone production in women can result in lowered bone density which is initially termed osteopoenia and then osteoporosis when the bone approaches the levels where it will fracture easily during normal activity. Bone loss affects the trabecular honeycomb bone more than the denser outer cortical bone and also the vertical struts more than the horizontal ones. Once one of the struts is broken it does not repair.

Because they are made up of large amounts of trabecular bones, the spinal bones are often badly affected by osteoporosis, with the process eventually allowing a buckling of the upper and lower surfaces of the vertebrae. The fronts of the vertebrae are more vulnerable to fracture as the trabeculae lose more mass in this area. Physiotherapists teach weight bearing exercises to counteract bone loss and drug therapies can halt breakdown and even increase bone mass. Getting our bone mass up to a high level during younger years so that the steady loss with age does not become important may be the best strategy.
